Get Nucleus Co-op
Download Nucleus Co-op, free for Windows 10 and 11, and unzip it into its own folder, for example C:\NucleusCo-op. There is no installer.
Keep it out of Program Files, your user folder and OneDrive.

Install the The Binding of Isaac: Repentance+ handler
In Nucleus Co-op, search the Hub for The Binding of Isaac: Repentance+ and install its handler, or download the handler file from this page and drop it on the Nucleus window. Then show Nucleus where The Binding of Isaac: Repentance+ is installed.

Take a seat
Each player presses a button on their own controller to join and takes a spot on the screen. Pick a layout that fits your players.
Playing with several keyboards and mice? Press End once everything is in place to give each window its own cursor.
Launch and connect
Press Launch: Nucleus opens one copy of The Binding of Isaac: Repentance+ per player and lays the windows out on your screen. Then link them in game, usually one player hosts a LAN or online session and the others join it. The handler notes say how.

Nucleus links the windows through the game's own LAN or online mode; the handler takes care of it.

Handler notes

For Binding of Isaac: Repentance+ online mode, make sure you have this DLC already installed in Steam, the game already has local co-op for 4 players but with this handler each player can get his own screen, useful for the game's big rooms or when using multiple monitors, you can also use multiple keyboards. Tested and working with latest legit Steam version as of February 26 2025.
Split-screen setup guide.
Create a public online match in one instance and join in the others. If you use multiple keyboards you have to remap the keyboard arrow keys to something else like i,j,k,l in the game options-controls as locking the input disables the arrows keys in this game for some reason, connect the instances with your main keyboard arrow keys by clicking or alt-tabbing between instances, once you are in-game lock the input by pressing the END key once so the keyboards restrict and work correctly, if you lock the input before all are in-game you will not be able to use the arrow keys in the main menu.
